Our Financial Policy
We
are participating providers in most managed care insurance plans. Please
refer to your insurance directory to determine if we are providers for
your specific plan. If your insurance requires you to have a referral to
see a specialist, it is your responsibility to obtain this from your
primary care physician. Co-pays and deductibles must be paid at the time
of your visit. With insurance plans for which we do not participate, we
will be happy to file your claim, but request full payment at the time
of your visit. Our office policy is to comply with standard coding
guidelines set by our coding specialists. Since our coding for insurance
claims is accurate by national standards, we are not able to change the
coding for greater reimbursement. Coverage problems should be discussed
with your insurance company. Our insurance department is always happy to
discuss your account with you if you ever have a concern.
Our
standard fees vary based on the level of service provided by your
provider. Surgical fees are based on usual and customary charges in our
area. Charges are also incurred for laboratory tests, ultrasound
examinations, and in-office procedures such as urodynamics, office hysteroscopy, or bone
densitometry. If an outside lab is used for tests or pathology, there
will be another charge from that facility. Most insurance policies cover
these procedures and we will gladly file the appropriate forms when a
service is rendered. If your insurance requires you to use a specific
lab, please advise the nurse at the time of your visit. Arrangements for
monthly payments can be made with the business office. We accept cash,
checks, Visa and MasterCard. Please call the office with any questions
